Applications from those seeking admission for their wards into the CRISTOBAL INTERNATIONAL SCHOOL, who are neither the father nor mother of the Child(ren) seeking enrolment, MUST delete the word PARENT and insert their own starts in regard to their ward.

This form MUST be accomppanied by:

1. four (4) recent passport photographs of the child.
2. Photocopy of the chil’s birth certificates and the original (for sighting)
3. Photocopy of the child’s immunization records and the original (for sighting)
4. Last result from former school
